SUPPLIER TERMS AND CONDITIONS OF PURCHASE

By accepting a purchase order from Qualitas Industries or any of its affiliates, the supplier hereby agrees to all terms and conditions listed below along with any other significant terms and conditions stipulated by the end user, which may change from time to time. Suppliers hereby agree to notify Qualitas Industries of nonconforming product or to obtain approval for disposition. Supplier also agrees to retain records for a minimum of 20 years.
